Source for DVOA Wallet Card?

    Hello!

    A few years ago, a DVOA member gave me this card at the Anza-Borrego meet:

    Front.   Back.

    --> Does anyone know who originally created this card, and if they might be willing to share the source file?

    I'd like to create something similar for RMOC.  It would be great if I could use this same format and just replace the logos, contact details, etc., rather than reinventing the wheel...

    Just in case anyone else is interested -- I got this reply from Kathy Urban on the DVOA email list:

    "I'm sure Dave Urban was handing out these cards at Anza Borrego, and Bob Gross made up the cards, I think."
